Videoconferencing Etiquette

This sheet has the guidelines for Site Managers as well as information to share with participants to familiarize them with the videoconferencing environment.


Network Use Form

For all videoconferences, participants need to sign in on the Network Use Form.  If you have received a sign-in sheet as the host of the event, please have participants sign in and fax the sheet back to 972.348.1515. This form also includes a videoconference waiver to make sure participants are aware they may be seen broadcast from site to site.


Participant Waiver

Anyone who participates in a videoconference must sign a Participant Waiver form. If they have participated in a previous videoconference, they do not need to sign another waiver. Note: Students must also sign a participant waiver. Participating in a videoconference may or may not be included in the district's Acceptable Use Policy.  When in doubt, send waiver forms home with students prior to videoconference for parental signature, videoconference activities and support.


VC Contact Update Form

Please use this form to update information regarding site contacts responsible for videoconference activities and support.


Evaluation Forms

If the videoconference is a Region 10-sponsored event, evaluations forms need to be completed.  Please fax them to 972.348.1515 at the conclusion of the videoconference.
