Student Distance Learning Waiver
Each student (or student's parent if underage) should sign a waiver before participating in a videoconference. If a student is in a credit-bearing class the district needs to retain only one signed waiver for the school year.

Participant Waiver / Network Use Form
For all videoconferences, participants need to sign in on the Network Use Form. This form also includes a videoconference waiver to make sure participants are aware they may be seen broadcast to all sites in the videoconference.
Please use this form to update information regarding site contacts responsible for videoconference activities and support.
If the videoconference is a Region 10-sponsored event, evaluations forms need to be completed. Please FAX them to 972.348.1515 at the conclusion of the videoconference.
